SERVICE AGREEMENT

This agreement made by and between company referred to as COMPLETE LIFESTYLE CONCIERGE and client.

Whereas, Complete Lifestyle Concierge is in the business of providing quality of life services, designed to create work-life balance and assistance of day-to-day tasks.

It is hereby agreed by and between the parties to adhere to as follows:

The services to be performed by Concierge are per request and by appointment. Because of the unique nature of services being provided by concierge, it cannot always predict or guarantee what clients final bill will be. This will be dependent upon the amount of it items spent performing services as well as the amount of expenses associated with service.

50% deposit of the hourly rate/project rate will be charged at time of booking. The remaining balance will be charged the day of service.  For hourly rates, all billable hours are to be measured and invoiced in increments of 10 with a one hour minimum.

Additionally, client will be billed for and shall pay the following expenses: postage, delivery charges, long distance phone charges, products or services provided by referred business & other expenses incurred by concierge while performing services on behalf of client. Concierge does not bill clients on behalf of referred business. 

Clients will receive itemized receipts following the completion of service.

Service request that are dangerous or illegal will not be considered. 

Concierge will make every effort to keep client information personal and confidential.

Cancellation of services should be made 24 hours in advance or a $15 fee will be assessed.

A fee of $25 will be assessed for all returned checks and payments.

This agreement shall become effective upon completion of first hour of service and shall continue until terminated as provided in this agreement. This agreement may be terminated by either party at any time. Each party reserves the right to terminate this agreement for any reason.

Concierge agrees to provide conscientious and competent services to all of its clients. Concierge services include referrals to other businesses and professionals., which are chosen based on their great reputation and are expected to provide the highest quality of service. However, Complete Lifestyle Concierge is not and cannot be liable for any unsatisfactory services provided by any company or individual. CONCIERGE MAKES NO WARRANTY OR GUARANTEE THAT ANY REFERRED BUSINESS WILL SATISFACTORILY PERFORM OR PROVIDE ITS CONTRACTED SERVICES. While concierge may recommend or suggest a business or professional to client, it is the client’s sole responsibility to determine if that business or professional fits the needs of the client.

CONCIERGE ACTS AS AN INDEPENDENT CONTRACTOR, AND NOT THAT OF AN EMPLOYEE OR EMPLOYER TO CLIENT.

If for any reason, an inevitable accident occurs, and concierge or client is unable to perform under terms of agreement, client will be refunded for any advanced payments made for all services that were not provided.

The interpretation and enforcement of this agreement shall be in correspondence with the law of the state of New York. Any changes made to this agreement must be done so in writing and signed by both parties.

This agreement acts in the benefit of both parties and their respective group. This document constitutes the entire agreement between the parties. This agreement may be modified only in writing and must be signed by both parties.
